The personal health information that we collect may include, for example, your name, date of birth, address, records of your visits to VOX Mental Health, and the care that you received during those visits.

To provide you with quality psychotherapy and counseling services.
To obtain payment for your treatment (from OHIP, WSIB, your private insurer, or others).
To plan, administer and manage our internal operations.

You have the right to access your personal health information and to request corrections if you believe there are errors or omissions. We will respond to your request as quickly as possible and no later than 30 days after receiving your request.
